Social Services can help with a variety of issues:
- Patients can self refer or refer relatives for a ‘care needs assessment’. This may result in carers, or a Package of Care (POC).
- Occupational Therapy (OT) assessments can also be requested for home modifications – ramps, showers, toilet raises, grip bars etc.
- Safeguarding concerns can also be raised here.
